SES & DWS Kindergarten Registration set for Monday, May 9

Both the Smithville Elementary School and DeKalb West School will have Kindergarten Registration for the 2022-23 school year on Monday, May 9 from 8:00 a.m. until 2:00 p.m. but at different locations.

DeKalb West School will have Kindergarten Registration at the school (DWS) but Smithville Elementary School’s Kindergarten Registration will be at the County Complex.

Please bring COPIES of the following documents to Registration: child’s birth certificate; Current, up-to-date, physical examination and immunization record (These must be on a Tennessee Department of Health Certificate of Immunization, which can be obtained from your doctor’s office or the Health Department); Proof of residency for DeKalb County (i.e. gas/electric bill); Your child (they will be tested on registration day); Children who register to attend kindergarten must turn five years old on or before August 15, 2022.

Meanwhile, Smithville Head Start Registration for the 2022-23 school year will be Monday, May 9 from 9:00 a.m. until? at the County Complex. Please bring COPIES of the following documents to Registration: child’s birth certificate or mothers copy; Income documents such as W2, 1040 tax return, SNAP benefits, foster care placement agreement or zero income. Current immunization record (These must be on a Tennessee Department of Health Certificate of Immunization, which can be obtained from your doctor’s office or our local Health Department) Children who register to attend Head Start must turn three years old on or before August 15, 2022. If you are unable to bring copies a picture can be taken and scanned. If you are unable to attend this day please call 615-597-5168 to schedule an appointment.

Pre-K Registration forms for the 2022-23 school year can be picked up on Thursday, May 19 from 8:30 a.m. until 2:00 p.m at Smithville Elementary. This will be done in a drive-through fashion. Completed registration paperwork will need to be returned to Michelle Burklow at the Board of Education office. COPIES of the following documents must be submitted with your registration forms in order for it to be complete: Certified copy of birth certificate, social security card (requested but not required), Current, up-to-date, physical examination and immunization record (These must be on a Tennessee Department of Health Certificate of Immunization, which can be obtained from your doctor’s office or the Health Department),Proof of residency for DeKalb County (i.e. gas/electric bill),- Proof of income for all household members. Children who register to attend Pre-K must turn four years old on or before August 15, 2022.
